The READMIT lite mobile phone application is the module that analyzes retrospective and real time clinical data in database to stratify readmission risk of patients. Most of these readmissions appear attributable to medical issues other than those responsible for the initial admission. READMIT lite is developed to predict patients readmission rate based on their medical record using the algorithm of Support Vector Machine (SVM) methods.
